Dear Players and Parents,

As part of our continuing effort to improve the coaching provided in the travel program, we are requesting that you complete the questionnaires shown below concerning your son or daughter’s soccer experience this season. Individual questionnaires will be kept confidential, and only the VP of Travel and the Director of Coaching will view the information. The Director of Coaching will use a summary of all the questionnaires to provide feedback to the coaches. A better picture of the coach’s performance over the season will be painted only if everyone on the team completes a questionnaire.
